This site is provided by The Centers for Medicare & Medicaid Services (CMS) to offer resource information for dialysis facilities and stakeholders about the End-Stage Renal Disease (ESRD) Quality Incentive Program (QIP). Information shown here does not constitute official CMS policy. CMS encourages stakeholders, advocates, and others to refer to the Final Rules located in the Federal Register.

As part of CMS’ ongoing efforts to improve the quality of services provided to beneficiaries with ESRD, the ESRD QIP supports safety, value, and quality care. The ESRD QIP incentivizes high-quality care in dialysis facilities by linking a portion of payment directly to facilities’ performance on quality care measures. The ESRD QIP will reduce payments to ESRD facilities that do not meet or exceed certain performance standards.

To perform this work, the ESRD QIP gathers performance data about dialysis facilities from Medicare reimbursement claims, CROWNWeb, and external sources such as the Center for Disease Control and Prevention’s (CDC) National Healthcare Safety Network (NHSN).

Dialysis Facility Compare is a related Medicare site where beneficiaries, caregivers, and others can find detailed information about Medicare-certified dialysis facilities. Individuals can use Dialysis Facility Compare to learn about the services and the quality of care that facilities provide; the website also contains resources for patients and family members who want to learn more about chronic kidney disease and dialysis.

ESRD QIP Secure Portal (via QualityNet)

ESRD networks and dialysis facilities use CROWNWeb to enter and submit patient and clinical quality of care data to CMS.

REMIS gathers information from various CMS databases and then compares that data to data in CROWNWeb.

Stakeholders can view information about the ESRD QIP in part through a secure portal on the QualityNet website. Individuals who are designated as an Authorized Point of Contact for each facility can use this to download information, access reports and communicate directly with CMS using a secure member login.

Accessing CROWNWeb, REMIS, and/or ESRD QIP

The Enterprise Identity Management system (EIDM) is a Centers for Medicare & Medicaid Services (CMS) application to request and maintain usernames, passwords and Multi-Factor Authentication for CROWNWeb, the ESRD QIP system, and other CMS systems.

New Users

Before accessing CROWNWeb, REMIS, and/or ESRD QIP (via the QualityNet secure portal), authorized users must register to receive a unique login and password. The registration process is administered on the CROWNWeb site and as described in the EIDM and QARM Quick Start Guide.

QualityNet requires Internet Explorer with JavaScript enabled on it.

Steps to enable JavaScript in Internet Explorer
  1. Click on the Tools menu option at the top of the page.
  2. Select Internet Options from the drop-down menu.
  3. From the dialog box, choose the Security tab.
  4. Click on Custom Level.
  5. Set Active Scripting to Enable.
  6. Click OK to close the custom settings dialog.
  7. Click OK to close the Options dialog.
