Extraordinary Circumstances Exceptions (ECE) Policy

Inpatient Psychiatric Facility Quality Reporting (IPFQR) Program

The Centers for Medicare & Medicaid Services (CMS) offers a process for participating Inpatient Psychiatric Facilities (IPFs) to request and for CMS to grant extensions or exceptions with respect to the reporting of required quality data when there are extraordinary circumstances beyond the control of the facility.

In the event of such circumstances, an Extraordinary Circumstances Exceptions Request Form (PDF-71 KB), with all required sections completed must be submitted. The facility may request consideration for an extension or exception of the requirement to submit quality data for one or more quarters. The form must be signed by the IPF’s Chief Executive Officer (CEO) or designee and submitted within 90 calendar days of the date that the extraordinary circumstances occurred.

The completed Extraordinary Circumstances Exceptions Request form should be submitted via one of the following methods:

Note: This process does not preclude CMS from granting exemptions or extensions to facilities when it is determined that an extraordinary circumstance, such as an act of nature, affects an entire region or locale. If CMS makes the determination to grant an exemption or extension to facilities in a region or locale, CMS will communicate this decision to facilities, vendors, and Quality Innovation Network-Quality Improvement Organizations (QIN-QIOs) through routine communication channels, including but not limited to issuing memos, emails and notices on QualityNet.

QualityNet requires Internet Explorer with JavaScript enabled on it.

Steps to enable JavaScript in Internet Explorer
  1. Click on the Tools menu option at the top of the page.
  2. Select Internet Options from the drop-down menu.
  3. From the dialog box, choose the Security tab.
  4. Click on Custom Level.
  5. Set Active Scripting to Enable.
  6. Click OK to close the custom settings dialog.
  7. Click OK to close the Options dialog.
