Ambulatory Surgical Center Quality Reporting (ASCQR) Program

Ambulatory Surgical Centers (ASCs) subject to the Centers for Medicare & Medicaid Services' (CMS) quality improvement initiatives must fulfill the reporting requirements of the ASCQR Program to receive their full annual payment update. Data submissions must be in the prescribed format. For the claims-based measures, Quality Data Codes must be submitted on the Medicare Claim Form 1500 (or electronic equivalent), and data for the Web-based measures must be submitted through the QualityNet Secure Portal and through the National Healthcare Safety Network (NHSN) by the deadlines established by CMS.

Measures Submitted via Web-Based Tools

All data and files—whether submitted by a provider or third-party data vendor acting on the provider’s behalf—must be transmitted via the QualityNet Secure Portal or, for measure ASC-8, via the NHSN. All files and data exchanged via the QualityNet Secure Portal are encrypted during transmission. Transmission via the Secure Portal is the only CMS-approved method for the electronic exchange of private healthcare quality data.

Vendors

A third-party vendor may submit data on a provider’s behalf if previously authorized by the ASC. Note: The ASC must complete the online registration process for the QualityNet Secure Portal. If an ASC consists of multiple facilities and would like to request authorization to submit the Web-based questions for more than one facility, they may elect to become a vendor on behalf of those facilities by contacting the support contractor at oqrsupport@hsag.com or (866) 800-8756.

It is the responsibility of each ASC to ensure that its data, as reported to CMS, are accurate.

QualityNet requires Internet Explorer with JavaScript enabled on it.

Steps to enable JavaScript in Internet Explorer
  1. Click on the Tools menu option at the top of the page.
  2. Select Internet Options from the drop-down menu.
  3. From the dialog box, choose the Security tab.
  4. Click on Custom Level.
  5. Set Active Scripting to Enable.
  6. Click OK to close the custom settings dialog.
  7. Click OK to close the Options dialog.
