Web-Based Data Collection

Hospitals participating in the Hospital Inpatient Quality Reporting (IQR) Program are required to complete Web-based Measure data collection. Data entry is achieved through an online data collection tool, available to hospitals and authorized vendors, on the QualityNet Secure Portal under Quality Programs > Hospital Quality Reporting: IQR, OQR, ASCQR, IPFQR, PCHQR > My Tasks > Manage Measures > View/Edit Structural/Web-Based Measures/Data Acknowledgement (DACA) > Inpatient Structural Measures/DACA.

Note: Web-based data cannot be submitted via an XML file.

Perinatal Care (PC-01) Measure

Hospitals participating in the IQR Program are required to complete the Web-based Measure questions quarterly. The Inpatient Web-based Measure, Elective Delivery, documents the number of patients with elective vaginal deliveries or elective cesarean sections at greater than or equal to (≥) 37 and less than (<) 39 weeks of gestation completed.

Hospitals participating in the IQR Program that do not deliver babies may file a Perinatal Care (PC-01) IPPS Measure Exception Form (CY 2019 Data Collection) PDF Fillable Form-309 KB (Updated 05/2019) for Perinatal Care (PC-01). Otherwise, hospitals that do not deliver babies must enter a zero (0) for the measure questions every quarter.

Data Accuracy and Completeness Acknowledgement (DACA)

Hospitals participating in the IQR Program for FY 2020 are required to complete the DACA between April 1 and May 15, 2019. Hospitals acknowledge all information submitted for the Hospital IQR Program is complete and accurate to the best of their knowledge at the time of submission.
