Facility-Specific Reports

CMS distributes facility-specific reports (FSRs) to facilities eligible to report the OIE measures. The FSR is an Excel workbook file, which includes information on the facility’s patient episodes, scores, state results, and national results for each OIE measure. Information included in the FSR released April 2019 will use claims data from July 1, 2017 through June 30, 2018, and will be reported on Hospital Compare beginning in summer 2019.

CMS has created a mock FSR for facilities who do not receive reports to reference. The mock FSR contains simulated facility-level data. Reasons why facilities may not receive an FSR are included in the FSR User Guide. The FSR mock-up and User Guide are provided below.

This file serves as an example of the FSRs that eligible facilities will receive in April 2019.
